La consultation infirmixe8;re est rxe9;alisxe9;e aprxe8;s la consultation initiale du chirurgien. Lors de cette consultation, est remis un DVD contenant des interviews reprenant les informations donnxe9;es ainsi que des txe9;moignages de patientes sur leur parcours de soin. Évaluation de l’information donnxe9;e par le chirurgien et par l’infirmixe8;re (huit items allant des techniques chirurgicales xe0; la convalescence). Évaluation de l’intxe9;rxea;t et de la comprxe9;hensibilitxe9;. Cotation de 0 xe0; 10.
Rxe9;sultats
Évaluation par questionnaire anonyme sur 110 femmes prises en charge entre fxe9;vrier et dxe9;cembre 2007. Taux de participation : 72,7 % (80/110), xe2;ge moyen : 51,5 ± 9,2 ans. L’information donnxe9;e est jugxe9;e utile (chirurgien : note supxe9;rieure xe0; 9,3/10 en fonction des items, infirmixe8;re : note supxe9;rieure xe0; 9,4) et comprxe9;hensible (note supxe9;rieure xe0; 8,7/note supxe9;rieure xe0; 8,9). L’information faite par l’infirmixe8;re est plus comprxe9;hensible que celle du chirurgien, quels que soient les items. La consultation infirmixe8;re est jugxe9;e trxe8;s utile (9,45). Soixante-dix pour cent des patientes ont appris des informations supplxe9;mentaires, par rapport xe0; la consultation du chirurgien. Cette consultation n’a pas d’influence directe sur le choix de la technique (5,45/10), la consultation du chirurgien restant prxe9;pondxe9;rante. Cinquante pour cent des femmes ont encore appris des xe9;lxe9;ments nouveaux grxe2;ce au DVD. En revanche, il n’a peu d’influence sur le choix de la technique (4,84/10). La consultation infirmixe8;re et le DVD sont bien conxe7;us car 80 % des femmes n’ont pas eu besoin de plus d’information. Les xe9;lxe9;ments prxe9;pondxe9;rants dans le choix des malades sont la consultation du chirurgien, puis la consultation infirmixe8;re, puis le DVD. L’indice global de satisfaction du dispositif mis en place est trxe8;s bon 9,31/10 (chirurgien : 9,24 ; infirmixe8;re 9,56, DVD 9,11).
Conclusion
La consultation infirmixe8;re et le DVD ne sont pas des xe9;lxe9;ments de substitution de la consultation du chirurgien. Ils sont complxe9;mentaires et ont un rôle important dans l’acquisition de l’information et le soutien psychologique des malades. La consultation initiale du chirurgien reste prxe9;pondxe9;rante dans la dxe9;cision de choix par les malades, elle doit rester la plus complxe8;te possible.
Summary
To evaluate the installation of a nurse consultation and an interactive support DVD in breast reconstruction. To judge utility of these new tools of information for the patients.
Material and method
The nurse consultation is carried out after the initial consultation of the surgeon; during this consultation, a DVD containing of the interviews is given taking again the information given as well as patients’ evidence on their path of care. Evaluation of the information given by the surgeon and the nurse (eight items from surgical techniques to convalescence). Evaluation of the interest and comprehensibility. Quotation from 0 to 10.
Results
Evaluation by anonymous questionnaire on 110 women, between February and December 2007. Rate of participation: 72.7 % (80/110); mean age: 51.5 ± 9.2. Information given is considered to be useful (surgeon: quotation above 9.3/10 in function of the items, nurse: quotation above 9.4) and comprehensible (quotation above 8.7/quotation above 8.9). The information made by the nurse is more comprehensible than that done by the surgeon, for over all items. The nurse consultation is considered to be very useful (9.45). Seventy percent of the patients learned from extra informations, compared to the consultation of the surgeon. This consultation does not have a direct influence on the choice of the technique (5.45/10), the consultation of the surgeon remaining predominant. Fifty percent of the women still learned from new informations, by using the DVD. On the other hand, it has little influence on the choice of the technique (4.84/10). The nurse consultation and the DVD are well conceived because 80 % of the women did not need more information. The predominant elements in the choice of the patients are the surgeon consultation, then the nurse consultation and then the DVD. The total index of satisfaction is very good 9.31/10 (surgeon: 9.24, nurse 9.56, DVD 9.11).
Conclusion
The nurse consultation and the DVD are not replacement components of the surgeon consultation. They are complementary and have an important role in the acquisition of information and the psychological support of the patients. The surgeon initial consultation remains predominent in the decision of choice by the patients, it must remain the most complete possible.
