Lieu de l’xe9;tude : centre de maternitxe9; et de nxe9;onatologie de Tunis (CMNT) (service de nxe9;onatologie, services de gynxe9;cologie obstxe9;trique A, B, et C).Patients et mxe9;thodes
Il s’agit d’une xe9;tude rxe9;trospective sur 18 nouveau-nxe9;s hospitalisxe9;s pour AIR dans le service de nxe9;onatologie du CMNT et ce entre le 1er janvier 2004 et le 31 dxe9;cembre 2006 (troisixe8;me pxe9;riode PIII) pour AIR. Nous avons ensuite comparxe9; nos rxe9;sultats xe0; ceux de deux autres xe9;tudes publixe9;es (Khouadja-Mokrani et al., 2005 [1], Debbiche et al., 1994 [2]) qui ont intxe9;ressxe9; 167 nouveau-nxe9;s hospitalisxe9; dans le mxea;me service entre le 1er janvier 1981 et le 31 dxe9;cembre 1990 (premixe8;re pxe9;riode P1) et 169 autres nouveau-nxe9;s hospitalisxe9;s entre le 1er janvier 1991 et le 31 dxe9;cembre 2003 (deuxixe8;me pxe9;riode PII).
Rxe9;sultats
L’incidence de l’incompatibilitxe9; fœto-maternelle dans le systxe8;me Rhxe9;sus a xe9;txe9; de 1,9 % des nouveau-nxe9;s hospitalisxe9;s au service au cours de la premixe8;re pxe9;riode contre 1,2 % au cours de la deuxixe8;me pxe9;riode et 0,28 % au cours de la troisixe8;me pxe9;riode ; il existe une tendance significative xe0; la baisse de l’incidence au cours des annxe9;es (χt 23,073 et p < 10–3). Au cours de la premixe8;re xe9;tude l’ictxe8;re prxe9;coce a xe9;txe9; le motif principal d’admission des nouveau-nxe9;s (80,2 % des cas). L’AIR a xe9;txe9; connue dans 15,5 % cas en antxe9;natal. Au cours de la deuxixe8;me pxe9;riode, la maladie a xe9;txe9; diagnostiquxe9;e en antxe9;natal dans 46,1 % des cas et l’ictxe8;re prxe9;coce a xe9;txe9; le motif d’hospitalisation pour 35,5 % cas. Au cours de la troisixe8;me pxe9;riode le diagnostic antxe9;natal a xe9;txe9; fait dans 44,4 % . Il existe une tendance significative xe0; la hausse du diagnostic antxe9;natal au fil des pxe9;riodes. Dans notre sxe9;rie le taux de faible poids de naissance a augmentxe9; bien que de faxe7;on non significative, au cours des trois pxe9;riodes. Par ailleurs les auteurs rapportent l’xe9;volution de la prise en charge antxe9;- et postnatale ainsi que l’xe9;volution de la morbiditxe9; et de la mortalitxe9; lixe9;e xe0; cette pathologie.
Conclusion
La meilleur prise en charge postnatale des AIR a permis de baisser le taux de mortalitxe9; globale lixe9;e xe0; cette pathologie. Nxe9;anmoins des efforts doivent xea;tre encore fournis xe0; plusieurs xe9;chelles, en particulier au niveau de la prxe9;vention de cette pathologie.
Summary
Aim
Compare the change of prevalence, clinical and therapeutic features in Rhesus alloimmunization, in a reference maternity which involves 8 % of the deliveries of the whole country.
Place of study
Center of maternity and neonatology of Tunis, Division on neonatology, A, B and C of gynecology and obstetrics.
Patients and methods
It is a retrospective study of 18 newborns which were hospitalized for rhesus alloimmunization (RAI) In the department of neonatology of the Center of maternity and neonatology of Tunis; during the period between 1st January 2004 and 31 December 2006 (third period).We have compared our results to those observed in articles which concerned 167 newborns during the period between 1st January 1981 and 31 December 1990 (first period) and 169 newborns during the period between 1st January 1991 and 31 December 2003 (second period).
Results
The incidence of feto-maternal incompatibility in the rhesus system is 1.9 % among newborns admitted in the department of the Center of maternity and neonatology of Tunis during the first period compared with 1.2 % in the second period and 0.28 % in the third period. We notice a significant decrease in the prevalence of RAI during these years (χt 23.073 and p < 10–3). During the first period, jaundice was the first cause of admission (80.2 % of the cases). The RAI was diagnosed antenatally in 15.5 % of the cases. During the second period; the antenatal diagnosis was made in 46.1 % and jaundice was the cause of admission in 35.5 % of the cases. During the third period, the antenatal diagnosis was made in 44.4 % . There is a significant increase in the antenatal diagnosis during the second and third period. In our study, the prevalence of low birth-weight has increased although the increase is not significant during these three periods. On the other hand, authors note the improvement postnatal management and the morbidity and mortality linked to this pathology.
Conclusion
The improvement postnatal management of RAI allowed to decrease the global rate of mortality linked to this pathology. However, many other efforts should be done in this field particular in the prevention of these diseases.
