We present the clinical findings of 4 cases of TNF-alpha inhibitor (TNFα-I) associated myelopathy. TNFα-I induced disease could present as myelopathy without brain involvement. We emphasize the importance of considering TNFα-I induced myelopathy in the differential diagnosis of transverse myelitis.
