•: CXCL12 in human brain was down-regulated in NMOSD patients with poor recovery.
•: CXCL12 was negatively related with the up-regulation of NMO-IgG in patients' serum.
•: CXCL12 was negatively related with the up-regulation GFAP in NMOSD patients' CSF.
