Postictal generalized EEG suppression is usually determined with scalp EEG (s-PGES).

Local brain activity in deeper brain structures may be missed.

Intracranial EEG was examined in generalized convulsive seizures (GCS) with s-PGES.

In most GCS, local brain activity persisted or reemerged before s-PGES resolved.

Potential implications for the pathophysiology of SUDEP are discussed.
