Symptom substitution has been a long-standing concern with behavior therapy.

Tic disorders provide an opportunity to empirically evaluate symptom substitution.
•
Behavior therapy and supportive therapy were used to treat 228 participants with tics.
•
Changes in tics, medications, comorbid symptoms, and adverse events were evaluated.
•
Results indicated that symptom substitution did not occur with behavior therapy for Tourette’s disorder.
