We present the clinical, radiologic and pathologic findings of two male adult patients, 38 and 52 years old, who underwent surgery in our department of a single medullar and a fourth ventricle lesion. Histological analysis was performed with the diagnosis of tanycytic ependymoma, characterized by fascicles of spindly bipolar tumor cells resembling tanycytes, perivascular pseudorosettes and absence of ependymal rosettes, Rosenthal fibers or mitoses. Positive glial fibrillar acid protein is typical. There is a tendency to observe this tumor in the spinal chord with a slightly better prognosis tan other ependymoma variants. Recommended treatment, including total resection, is similar to current surgical practice for other grade ii ependymomas.
