New good practice guidelines for diagnosis and antibiotherapy of community-acquired bacterial urinary tract infections (including uncomplicated acute cystitis) have been established by the French Health Products Safety Agency (AFSSAPS) and the French Association of Urology (AFU).
Limiting the use of fluoroquinolones in lower urinary tract infections without severity signs should be considered in order to preserve their efficacy in ¡°complicated?lower urinary tract infections, upper urinary tract infections and male genital tract infections.
The management of uncomplicated acute cystitis in women has to be the same regardless of their age and hormonal status, based on a probabilistic treatment:
- -
first line: fosfomycin trometamol in a single dose;
- -
second line: nitrofurantoin (5 days); fluoroquinolone in a single dose or for 3 days.
