Three CADASIL patients are presented, all initially thought to have MS. The McDonald criteria for MS highlight the need to exclude other conditions. The clinical and MRI red flags to suggest a diagnosis of CADASIL are discussed. There is no current disease modifying treatment available for CADASIL. CADASIL patients and their family members should be referred for genetic counselling.
