•: We examined the relation between leg composition and V_E/VCO₂ during exercise in HF.
•: V_E/VCO₂ was reduced with afferent inhibition (fentanyl) during submaximal exercise.
•: The reduction in V_E/VCO₂ was strongly associated with leg muscle and fat mass.
•: Leg fat mass was predictive of the reduced V_E/VCO₂ slope with afferent blockade.
•: Greater reductions in V_E/VCO₂ were observed in patients with the lowest peak VO₂.
