Serous cystic neoplasms of the pancreas are considered as benign tumors. However, 30 cases of pancreatic serous cystadenocarcinoma have been published suggesting that they carry a realistic risk of malignancy. We report the case of a 79 year-old woman who was hospitalized for upper digestive haemorrhage on a neoplasm of the head of the pancreas. There was no metastasis on the contrasted-enhanced body CT scan but some were seen in the liver during a upper echo-endoscopy and were biopsied. Their histopathological examination was non-contributive and their aspect in MRI was in favor of focal nodular hyperplasia. The patient underwent a pancreaticoduodenectomy with biopsies of the liver’s metastasis and the histopathological examination revealed a serous cystadenocarcinoma.
