Airway management in oral and maxillofacial surgery with the patient under general anesthesia is especially demanding. The anatomical airway structures can be compromised by congenital malformations and acquired pathologies. Therefore, a thorough preoperative patient evaluation by an experienced anesthesiologist is mandatory in order to find the safest strategy for securing the airway. Airway management is only successful with trained interdisciplinary cooperation and a differentiated application of the various techniques. Fiberoptic intubation of the awake patient remains the gold standard for difficult airways. In cases of an unexpected difficult airway, an experienced anesthesiologist can in most cases establish a secure airway by using modern devices, such as video laryngoscopes.
