摘要
目的探讨腔内修补术联合股-股动脉转流"杂交"技术治疗特殊腹髂主动脉病变的疗效。方法对3例特殊腹髂主动脉病变患者采取了腔内修补联合股-股动脉转流"杂交"技术治疗,术后观察主动脉内瘘、股-股动脉转流人工血管通畅性以及下肢供血情况。结果围术期均无死亡,术后1、3、6个月均复查腹主动脉CTA,第1例腹主动脉Ⅱ型内瘘仍存在,但无明显进展表现;第2例、第3例无内瘘形成。3例患者的随访时间为6~28个月,在随访过程中旁路人工血管均畅通,均未出现吻合口狭窄、下肢缺血症状。结论对于某些特殊的腹髂主动脉病变,采取腔内修补联合股-股动脉转流"杂交"技术治疗是可行、安全、有效的,但其远期疗效仍需长期随访。
Objective To evaluate the efficacy of hybrid technique of endovascular aneurysm repair( EVAR) combined with cross femoral bypass grafting( CFBG) for the treatment of special abdominal iliac aortic disease. Methods Three patients with special abdominal iliac aortic disease were treated with hybrid technique of EVAR combined with CFBG. The aortic internal leakage,artificial vascular patency of the bypasses and blood supply of the lower limbs were observed after the operation. Results No perioperative death occurred. The abdominal aorta CTA was conducted. At 1,3,and 6 months after operation,aortic internal leakage of type Ⅱwithout obvious progression was remained in the first case. No internal leakage was found in the second and third cases. The three patients were followed up for 6-28 months. During the follow-up,artificial vascular of the bypasses was patent,and no anastomotic stricture and ischemia of the lower limbs were found. Conclusion For special abdominal iliac aortic disease,the hybrid technique of EVAR combined with CFBG is feasible,safe and effective,but the long-term efficacy should be assessed by follow-up.
