摘要
目的探讨超声心动图在冠脉支架植入术患者术后随访中的应用价值。方法选取2016年7月至2017年7月上饶市立医院和江西省人民医院收治的行单支狭窄冠脉支架植入术的50例冠状动脉性心脏病(CHD)患者作为研究对象,在冠脉支架植入术后6~12个月进行随访,同时进行超声心动图检查和冠脉造影检查。通过超声心动图检查观察患者的室壁运动积分指数(WMSI)、收缩期心内膜的向心运动幅度以及室壁增厚率等室壁运动指标在手术前后的变化情况。并将超声心动图的检查结果与冠脉造影检查结果进行对比,分析超声心动图在术后随访中的应用价值。结果超声心动图检查结果显示50例患者在冠脉支架植入术后的WMSI低于手术前,而收缩期心内膜的向心运动幅度以及室壁增厚率均较手术前明显增加,差异均有统计学意义(P均<0.05)。超声心动图检查显示运动异常好转37例,诊断符合率为86.49%;运动异常无明显改变8例,诊断符合率87.50%;出现新的室壁运动异常节段5例,诊断符合率80.00%。结论运用超声心动图能较好的评估冠脉支架植入术的疗效,且操作方便,安全性好,在患者术后随访中具有较高的应用价值。
        Objective To evaluate the value of echocardiography in the follow-up of patients undergoing coronary stenting. Methods From July 2016 to July 2017, 50 patients with coronary artery disease underwent singlevessel coronary stent implantation were selected as the study subjects.The patients were followed up from 6 months to 12 months after coronary stent implantation. Echocardiography and coronary angiography were performed. Echocardiographic examination was performed to observe the changes of the wall motion integral index(WMSI),the concentric motion amplitude of systolic endocardium and the wall thickening rate of the patients before and after operation.The results of echocardiography were compared with the results of coronary angiography, and the value of echocardiography in postoperative follow-up was analyzed. Results Echocardiography showed that the WMSI of 50 patients after coronary stent implantation was lower than that before coronary stent implantation, while the amplitude of centripetal movement and the thickening rate of ventricular wall in systolic period were significantly higher than those before coronary stent implantation(P < 0.05). Echocardiography showed that the abnormal motion was improved in 37 cases, the diagnostic coincidence rate was 86.49%(32/37), there were no obvious changes in motor abnormalities in 8 cases, and the diagnostic coincidence rate was 87.50%. New abnormal segment of ventricular wall motion was found in 5 cases, and the diagnostic coincidence rate was 80.00%. Conclusion Echocardiography can be used to evaluate the curative effect of coronary stent implantation, and it is easy to operate and safe. It has high application value in postoperative follow-up.
