摘要
目的观察稳心颗粒治疗频发房性期前收缩(PAC)的临床疗效。方法选取2016年3月—2017年4月陕西省友谊医院心内科收治的频发PAC患者62例,采用随机数字表法分为对照组32例与观察组30例。对照组患者采用普罗帕酮治疗,观察组患者在对照组基础上加用稳心颗粒治疗;两组患者均连续治疗4周。比较两组患者临床疗效,治疗前后QT间期离散度(QTd)、校正的QT间期离散度(QTcd)及心率变异性(HRV)时域指标,观察两组患者治疗期间不良反应情况及血常规、肝肾功能异常情况等。结果观察组患者临床疗效优于对照组(P<0.05)。治疗前两组患者QTd、QTcd比较,差异无统计学意义(P>0.05);治疗后观察组患者QTd、QTcd低于对照组(P<0.05)。治疗前两组患者连续24 h全部窦性心搏R-R间期标准差(SDNN)、每5 min窦性心搏R-R间期均值标准差(SDANN)、相邻窦性心搏R-R间期差值的均方根(RMSSD)、24 h内相邻R-R间期差值大于50 ms的个数所占的百分数(PNN50)比较,差异无统计学意义(P>0.05);治疗后观察组患者SDNN、SDANN、RMSSD、PNN50高于对照组(P<0.05)。两组患者治疗期间不良反应发生率比较,差异无统计学意义(P>0.05),血常规、肝肾功能等均未见明显异常。结论稳心颗粒治疗频发PAC的临床疗效确切,可有效降低QTcd,改善HRV时域指标,且安全性较高。
Objective To observe the clinical effect of Wenxin Granules on frequent premature atrial contraction. Methods From March 2016 to April 2017, a total of 62 patients with frequent premature atrial contraction were selected in the Department of Cardiology, Friendship Hospital of Shaanxi Province, and they were divided into control group(n=32) and observation group(n=30) according to random number table. Patients in control group received propafenone, while patients in observation group received Wenxin Granules based on that of control group; both groups continuously treated for 4 weeks. Clinical effect, QTd, QTcd and time-domain index of heart rate variability(HRV)(including SDNN, SDANN, RMSSD and PNN50) before and after treatment were compared between the two groups,and incidence of adverse reactions,blood routine examination results,liver and kidney function examination results were observed during treatmet. Results Clinical effect in observation group was statistically significantly better than that in control group(P<0.05).No statistically significant differences of QTd or QTcd was found between the two groups before treatment(P>0.05), while QTd and QTcd in observation group were statistically significantly lower than those in control group after treatment(P<0.05). No statistically significant differences of SDNN, SDANN, RMSSD or PNN50 was found between the two groups before treatment(P>0.05), while SDNN, SDANN, RMSSD and PNN50 in observation group were statistically significantly higher than those in control group after treatment(P<0.05). No statistically significant differences of incidence of adverse reactions was found between the two groups during treatment(P>0.05),meanwhile no one in the two groups occurred abnormal blood routine examination results,liver or kidney function examination results. Conclusion Wenxin Granules has certain clinical effect in treating patients with frequent premature atrial contraction, can effectively reduce QTcd and improve the time-domain index of HRV,and is relatively safe.
