摘要
目的:通过统计分析武汉大学人民医院儿童呼吸道常见病毒的感染率和感染型别与性别、年龄、季节等的关系,为儿童呼吸道病毒感染的诊断与预防提供临床依据。方法:采用直接免疫荧光法对2017年11月1日至2018年10月31日在武汉大学人民医院就诊的11 744例儿童呼吸道感染患者进行腺病毒(ADV)、流感病毒A、B(IFA、IFB)、副流感病毒1、2、3(PIV1、PIV2、PIV3)及呼吸道合胞病毒(RSV)7种呼吸道常见病毒抗原检测,并对其结果进行分析。结果:在11 744例患者中病毒感染阳性率为19. 20%,其中ADV 7. 58%、IFA 17. 78%、IFB10. 91%、PIV1 2. 35%、PIV2 1. 55%、PIV3 17. 03%、RSV 42. 48%、混合感染率为0. 31%。其中男、女PIV3感染阳性率差异有统计学意义(P=0. 017),但在其他病毒感染方面差异无统计学意义(P>0. 05)。各年龄段感染差异具有统计学意义(P<0. 05),0~岁与1~岁年龄段感染病毒的阳性均高于4~岁和7~14岁组,以RSV的感染阳性率最高。7种病毒在春、冬季感染阳性率均高于夏、秋季,差异具有统计学意义(P<0. 05),且冬春季均以RSV感染阳性率最高。结论:RSV是本院儿童呼吸道病毒感染的主要病原体,3岁以下儿童阳性率最高,冬、春季为感染的高发季节。
        Objective: To investigate the relationship between respiratory viral infection and sex, age and season in children in Hubei Province, and to provide clinical basis for the diagnosis and prevention of respiratory virus infection in children.Methods: 11 744 cases were collected in Renmin Hospital of Wuhan University from November 1, 2017 to October 31, 2018. Adenovirus(ADV), influenza virus A, B(IFA, IFB), Parainfluenza virus 1, 2, 3(PIV1, PIV2, PIV3) and respiratory syncytial virus(RSV) were detected by direct Immunofluorescence method, and results were analyzed.Results: The positive rate of viral infection was 19. 2% in 11 744 patients, including ADV 7. 58%, IFA 17. 78%,IFB 10. 91%, PIV1 2. 35%, PIV2 1. 55%, PIV3 17. 03%, RSV 42. 48%, mixed infection0. 31%. There were statistically significant differences in the positive rates of PIV3 infection between male and female(P=0. 017), but no statistically significant difference in other viral infections were found(P>0. 05). The difference of infection in all ages groups was statistically significant(P<0. 05).The positive rate of virus infection in the age groups of <1 year and 1-3 year(s) was higher than that in the groups of 4-6 years and 7-14 years, with the highest positive rate of RSV infection. The positive rate of 7 kinds of virus infection in spring and winter was higher than that in summer and autumn, the difference was statistically significant(P<0. 05) and the positive rate of RSV infection was the highest in winter and spring.Conclusion: RSV is the main pathogen of respiratory virus infection in children in Hubei province, and the positive rate of children under 3 years old is the highest, and winter and spring are the high-incidence seasons of infection.
