摘要
目的:探讨不停跳冠脉搭桥术(OPCABG)发生急性肾功能损伤(AKI)的相关危险因素。方法:将259例择期行OPCABG的患者分为AKI组和非AKI组,对术后发生AKI的可能变量单因素分析,对于P<0.05的变量进行logistic回归分析得到术后发生AKI的独立危险因素。结果:该研究共纳入259例行OPCABG的患者,术后AKI的发生率为30.5%。单因素分析结果:术后AKI与年龄、体质指数、合并脑血管病、术前尿酸值、手术时间有关联(P<0.05)。多因素回归分析结果:年龄、体质指数、术前尿酸、术前肌酐、手术时间为术后并发AKI的独立危险因素。结论:高龄、体质指数较高患者,注意对肾功能的保护,改善术前尿酸及肌酐水平、缩短手术时间等可减少术后AKI的发生。
        Objective:To investigate the risk factors of acute kidney disease(AKI) after off-pump Coronary artery bypass grafting(OPCABG). Methods:259 patients who underwent elective OPCABG were divided into two groups:AKI group, non-AKI group Univariate analysis was performed on the variables related to postoperative AKI in the two groups. Logistic regression analysis of variables with P<0.05 showed independent risk factors for AKI after OPCABG.Results:A total of 259 patients were included in the study. The incidence of postoperative AKI was 305.5%in 259 patients. Univariate analysis showed that AKI was associated with age, body mass index, cerebrovascular disease, preoperative uric acid value, and operation time after OPCABG(P<0.05). Multivariate regression analysis showed age, body mass index, preoperative uric acid, preoperative creatinine; operative time is an independent risk factor for concurrent AKI after OPCABG. Conclusion:For elderly patients with high body mass index, attention should be paid to the protection of renal function, improvement of preoperative uric acid and creatinine levels, and shortening of operation time when conditions permit, which may reduce the occurrence of postoperative AKI.
