摘要
目的分析合并感染华支睾吸虫的乙型肝炎患者的体液免疫水平的影响。探讨体液免疫应答在合并感染华支睾的乙型肝炎患者机体的防御反应机制。方法选取70例健康人、45例单纯华支睾吸虫感染患者、70例单纯乙肝患者、47例合并感染华支睾吸虫的乙型肝炎患者血清,运用免疫透射比浊法分别定量检测。结果乙型肝炎组比健康组的IgG、IgA和IgM均高,C3和C4均低,华支睾吸虫组和混合感染组分别与健康组相比,分别为IgG和C4降低,以及C3和C4降低,差异有统计学意义(P<0.05);同时,与华支睾吸虫组相比,乙型肝炎组IgG、IgA和IgM升高幅度更大, C3和C4均降低,两组比较差异有统计学意义(P<0.05);与乙型肝炎组相比,混合感染组IgG、IgA和IgM降低, C3和C4均升高,差异有统计学意义(P<0.05)。结论合并感染华支睾吸虫会降低乙型肝炎患者的体液免疫应答趋势,减弱补体的激活,由体液免疫和细胞免疫的协同作用转变为细胞免疫为主,体液免疫为辅,免疫机制进行转变,结果显示临床护肝治疗的同时进行驱虫治疗,可用来指导临床用药及选择正确的治疗方案。
        Objective To understand the humoral immunity of hepatitis B patients coinfected with Clonorchis sinensis and to explore the defense response mechanism of humoral immunity. Methods Serum levels of IgG, IgA, IgM, C3 and C4 were measured in 70 healthy individuals, 45 patients with Clonorchis infection, 70 patients with hepatitis B, and 47 patients with hepatitis B coinfected with Clonorchis sinensis using immune transmission turbidimetry method. Results Compared to healthy controls, the levels of IgG,IgA and IgM in patients with hepatitis B and coinfected with Clonorchis sinensis increased, while the levels of C3 and C4 decreased, the differences were statistically significant(P<0.05).There were significant differences in C3 and C4 levels between patients with coinfection and healthy controls(P<0.05).Compared to patients with hepatitis B, the serum levels of IgG, IgA, and IgM in patients with coinfection increased, while the levels of C3 and C4 decreased, the difference was statistically significant(P<0.05). Conclusions Coinfection with Clonorchis sinensis reduces the humoral immune response in patients with hepatitis B and weakens complement activation.The synergistic effect of humoral immunity and cellular immunity is transformed into cellular immunity, supplemented by humoral immunity, and changes in immune mechanisms and prompt clinical liver protection treatment while deworming treatment.
