二维斑点追踪成像技术对原发性高血压患者左室功能的诊断价值研究

摘要
目的评价二维斑点追踪成像技术(2D-STI)对原发性高血压患者左室功能的诊断价值。方法 36例原发性高血压患者,根据左室质量指数(LVMI)不同分为非肥厚组(20例)和肥厚组(16例)。另选取24例健康者作为对照组。应用二维斑点追踪成像技术分析三组左室整体纵向应变(LS)、径向应变(RS)、圆周应变(CS)情况。比较三组舒张期左房前后径(LADd)、LVMI、室间隔厚度(IVST)、左室后壁厚度(LVPWT)、左室舒张早期最大血流速度(E峰)/二尖瓣心房收缩期最大血流速度(A峰)(E/A)、左室射血分数(LVEF)水平。结果非肥厚组与对照组的LVMI、IVST、LVPWT、E/A、LVEF水平比较,差异均无统计学意义(P>0.05);非肥厚组LADd水平为(31.67±3.52)mm,大于对照组的(27.32±4.21)mm,差异具有统计学意义(P<0.05)。肥厚组与非肥厚组的LADd、LVEF水平比较,差异均无统计学意义(P>0.05);肥厚组LVMI、IVST、LVPWT大于非肥厚组, E/A小于非肥厚组,差异均具有统计学意义(P<0.05)。非肥厚组与对照组的RS、CS值比较,差异均无统计学意义(P>0.05);非肥厚组LS值为(-18.32±2.46)%,低于对照组的(-25.26±3.76)%,差异具有统计学意义(P<0.05)。肥厚组LS、RS、CS值均低于非肥厚组和对照组,差异均具有统计学意义(P<0.05)。结论运用二维斑点追踪成像技术能早期发现原发性高血压患者左室收缩功能的改变。
        Objective To evaluate the diagnostic value of two-dimensional speckle tracking imaging(2 D-STI) technique on left ventricular function of patients with essential hypertension. Methods A total of36 patients with essential hypertension were divided by different left ventricular mass index(LVMI) into nonhypertrophic group(20 cases) and hypertrophic group(16 cases). Another 24 healthy persons were taken as control group. The global longitudinal strain(LS), radial strain(RS) and circumferential strain(CS) of the left ventricle in three groups were analyzed by two-dimensional speckle tracking imaging technique. The left atrial end-diastolic diameter(LADd), LVMI, interventricular septal thickness(IVST), left ventricular posterior wall thickness(LVPWT),left ventricular peak early diastolic flow velocity(E peak)/mitral atrial systolic maximum flow velocity(A peak)(E/A), left ventricular ejection fraction(LVEF) levels were compared among the three groups. Results Nonhypertrophic group and hypertrophic group had no statistically significant difference in LVMI, IVST, LVPWT, E/A and LVEF(P>0.05). Non-hypertrophic group had larger LAEd level as(31.67±3.52) mm than(27.32±4.21) mm in hypertrophic group, and the difference was statistically significant(P<0.05). Non-hypertrophic group and hypertrophic group had no statistically significant difference in LADd and LVEF(P>0.05). Hypertrophic group had larger LVMI, IVST and LVPWT than non-hypertrophic group, and smaller E/A than non-hypertrophic group. Their difference was statistically significant(P<0.05). Non-hypertrophic group and control group had no statistically significant difference in RS and CS value(P>0.05). Non-hypertrophic group had lower LS value as(-18.32±2.46)% than(-25.26±3.76)% in control group, and the difference was statistically significant(P<0.05). Hypertrophic group had lower LS, RS and CS value than non-hypertrophic group and control group,and the difference was statistically significant(P<0.05). Conclusion Two-dimensional speckle tracking imaging technology can detect early changes in left ventricular systolic function in patients with essential hypertension.
