摘要
目的研究不同孕周胎儿第12对胸神经与脊髓主干夹角的变化关系,丰富胎儿脊柱脊髓发育的相关参数,为胎儿脊柱脊髓及脊神经发育异常的产前诊断提供依据。方法选取38例不同孕周的正常发育胎儿标本,行3.0T高场磁共振扫描,以第十二肋骨为参照确定椎间孔位置,进而确定第十二对脊神经,测量其与脊髓主干夹角,将所测量数据与孕周做相关统计处理。结果第12对胸神经与脊髓主干夹角在孕中晚期随孕周增加而减小,呈线性相关,其相关方程为:右侧所测角度(°)=49.327-0.704×孕周,R2=0.965,P<0.05;左侧所测角度(°)=48.265-0.67×孕周,R~2=0.952,P<0.05。所测角度在左右两侧及性别间无统计学差异。结论 3.0T高场磁共振可清晰显示脊柱、脊髓、脊神经等相关结构,并可清晰观察到脊神经走行,第十二对脊神经与脊髓主干夹角随孕周增加而减小,丰富了胎儿脊柱脊髓发育的相关图谱及解剖数据。
        Objective To study the relationship of the included angle of the twelfth pairs of thoracic nerve and the main trunk of spinal cord in fetus of different gestational weeks,to enrich the database which included many related parameters about the development of fetal spinal cord,and to provide the basis for the dysplasia of fetal spinal cord and spinal nerve in prenatal diagnosis.Methods 38 of specimens from normal fetuses at different gestational weeks were being scanned with 3. 0 T high field magnetic resonance. Then,we selected the twelfth rib as a reference to determine the location of intervertebral foramen,and then determine the twelfth of spinal nerve. Finally,we measured the trunk angle between the spinal cord and the twelfth of spinal nerve,and did statistical research about the data and gestational weeks. Results The angle decreases with the increasing gestational age at the second and third trimester,showing a linear correlation. The correlation equation is as follows: the right angle( °) = 49. 327-0. 704 × gestational weeks,R~2= 0. 965,P < 0. 05; the left angle( °) = 48. 265-0. 67 × gestational weeks,R2= 0. 952,P <0. 05. There are no statistically significant differences in angle of either side and in gender. Conclusion The spine,spinal cord,spinal nerve and other related structures can be shown clearly by 3. 0 T high field MRI and the spinal nerves can be observed clearly. The trunk angle between the spinal cord and the twelfth of spinal nerve decreases with the increasing gestational age. The study enriches the anatomic map and the related data about the development of fetal spinal cord.
