摘要
目的:探讨脑卒中神经源性膀胱个案管理的应用效果。方法:收集2014年1月至2016年12月在西南医科大学附属医院康复科首次脑卒中住院、并符合纳入标准的患者共152例,随机分为干预组与对照组,每组各76例,对照组给予出院指导和健康教育,干预组按照个案管理方案进行持续干预6个月。结果:两组患者在出院后6个月在并发症发生率、复诊率、居家清洁导尿操作正确率、日常生活能力(activity of daily living,ADL)评分、尿失禁生活质量问卷评分方面组间比较,差异有统计学意义(P<0.05);但在再住院率方面,两组比较差异无统计学意义(P>0.05)。结论:对脑卒中神经源性膀胱患者进行个案管理能有效改善患者出院后6个月内的临床结局指标,具有一定的临床应用意义。
Objective: To explore the effect of case management on neurogenic bladder in stroke. Method: A total of 152 cases of hospitalized patients with the first stroke from January 2014 to December 2016 were randomly divided into an intervention group and a control group with 76 cases in each group. The patients in the control group were given discharge guidance and health education, and the patients in the intervention group were given the case management for intervention with 6 months. Results: Two groups of patients discharged from the hospital after 6 months, the incidence rate, return rate, clean intermittent catheterization operation rate and ADL score,quality of life questionnaire urinary incontinence score between the groups were statistically significant difference in complications(P<0.05); but in the readmission rate of two group had no significant difference(P>0.05).Conclusion: Case management of neurogenic bladder in patients with stroke can effectively improve the clinical outcome within 6 months after hospital discharge, and can be a recommendation for clinical significance.
