摘要
目的:观察无缝线小切口手法碎核白内障摘除人工晶状体植入的手术效果。方法:随机选择防盲工程中白内障患者102例102眼,根据患者手术方法分为两组,观察组51例51眼,采用无缝线改良小切口手法碎核白内障手术,植入人工晶状体;对照组51例51眼,行有缝线小切口手法碎核白内障摘除,植入人工晶状体。分别观察术后1 d、1周、1个月裸眼视力,术后1个月角膜散光值和最佳矫正视力。结果:术后1 d、1周和1个月,观察组的裸眼视力和术后1个月最佳矫正视力优于对照组,差异有统计学意义(P<0.05)。观察组术中术后角膜水肿、后囊破裂等并发症发生率均较对照组少,观察组术后1个月角膜散光值低于对照组,差异有统计学意义(P<0.05)。结论:对白内障患者实施无缝线改良小切口手法碎核白内障手术,可有效减少术后角膜散光,提高术后裸眼视力,减少术后并发症,具有一定的实用性和有效性,适合在基层防盲中应用。
Objective:To assess the clinical effect of modified small incision on sutureless manual nucleofragmentation cataract surgery for intraocular lens implantation(ILI) in blindness prevention program.Methods:One hundred and two patients with cataract(102 eyes) were randomized into observational group(n=51,51 eyes,treated with modified small incision in sutureless manual nucleofragmentation cataract extraction with ILD) and control group(n=51,51 eyes,treated by small incision in suture manual nucleofragmentation cataract extraction with ILD).The two groups were observed regarding postoperative visual acuity recovery the next day,one week and one month after surgery,respectively,as well as corneal astigmatism and optimally corrected visual acuity one month following operation.Results:Patients in the observational group had better naked visual acuity the next day,one week and one month after surgery,and corrected visual acuity one month following procedure than those in the control group.The difference was significant(P<0.05).Fewer complications including corneal edema and posterior capsule rupture and better naked visual acuity one month after operation were found in patients in the observational group than in the control group(P<0.05).Conclusion:Modified small incision in sutureless manual nucleofragmentation cataract surgery for intraocular lens implantation can be applicable in the blindness prevention program in medical institutions at grass-root level,because this technique may reduce the corneal astigmatism and postoperative complications,yet improve the naked visual acuity as well as be workable and effective.
