摘要
目的观察托补促愈汤冲洗联合负压封闭引流技术治疗骨科创面感染的临床疗效。方法选取2017年6月—2018年10月就诊的骨科创面感染患者70例,采用随机数字表法分为对照组与治疗组,每组35例。2组均首先进行常规清创,然后采用VSD敷料覆盖创面,最后连接负压吸引设备进行创面冲洗、持续吸引。对照组以0. 9%氯化钠注射液为冲洗液,治疗组以托补促愈汤为冲洗液。2组均治疗2周。结果治疗组总有效率高于对照组(P <0. 05)。2组治疗后创面渗出液IL-6、TNF-α水平及创面疼痛、创面肉芽评分均低于治疗前(P <0. 05),且治疗组低于对照组(P <0. 05)。结论托补促愈汤冲洗联合VSD治疗骨科创面感染,能够减轻创面炎症反应及疼痛程度,促进肉芽组织生长及创面愈合,提高临床疗效。
Objective To observe the clinical efficacy of Tuobu Cuyu decoction washing combined with vacuum sealing drainage(VSD) in the treatment of orthopedic wound infection. Methods Seventy patients with orthopedic wound infections who were treated in our hospital from June 2017 to May 2018 were selected and divided into control group and treatment group by random number table,with 35 cases in each group. The two groups were first routinely debrided,then the VSD dressing covered the wound surface,and finally the vacuum suction device was connected for wound washing and continuous attraction. The control group was rinsed with 0. 9 %sodium chloride injection,and the treatment group was treated with Tuobu Cuyu decoction. Both groups were treated for 2 weeks.Results The total effective rate of the treatment group was higher than that of the control group(P < 0. 05). The levels of IL-6 and TNF-α in wound exudates and wound pain and wound granulation scores in the two groups were lower than those before treatment(P <0. 05),and the treatment group was lower than the control group(P < 0. 05). Conclusion Tuobu Cuyu decoction combined with VSD in the treatment of orthopedic wound infection can reduce the inflammatory reaction and pain level of wounds,promote granulation tissue growth and wound healing,and improve clinical effect.
