小柴胡汤治疗大鼠子宫内膜异位症的实验研究

摘要
目的:采用大鼠子宫内膜异位症(Endometriosis,EMS)动物模型,研究中药制剂小柴胡汤(Xiao Chai Hu Tang,XCHT)对EMS大鼠异位内膜(Endometriotic Tissue,EOS)的影响及其可能的机制。
     方法:(1)采用自身子宫内膜移植法建立大鼠EMS模型,分组给予不同剂量XCHT、丹那唑(Danazol)治疗4周。(2)通过观察、测量EOS治疗前后体积,了解药物对EOS的作用。(3)医学图像定量分析EOS子宫内膜上皮高度(H)、腺细胞核体积(V)、核体积积分光密度(Viod)。(4)透射电子显微镜下观察各组在位子宫内膜(Endometrium,EM)、EOS超微病理学特征。(5)免疫组化观察、分析各用药组EM及EOS的Caspase-3、Fas蛋白表达特征。
     结果:(1)成功建立了大鼠EMS模型。(2)与对照组比较,中、高剂量XCHT组及丹那唑组对EOS的抑制作用明显,EOS体积较治疗前明显减小(P<0.01)。(3)医学图像定量分析结果显示,中、高剂量XCHT组及丹那唑组子宫内膜上皮高度、核体积、核体积积分光密度较对照组明显减小(P<0.01)。(4)透射电镜下可见中、高剂量XCHT组治疗后较多腺上皮细胞出现凋亡特征,表现为细胞体积变小、核固缩、胞浆和核染色质凝集,密度增高,细胞间凋亡小体,同时基质细胞中可见一些坏死细胞。(5)免疫组化结果显示,对照组EM中凋亡相关蛋白Caspase-3、Fas的表达水平明显高于EOS;而XCHT组恰恰相反,丹那唑组差异不明显。
     结论:(1)EMS发生、发展与EOS本身细胞凋亡水平异常有关。(2)XCHT对EMS大鼠EOS的生长、增殖具有抑制作用。(3)XCHT治疗EMS的机理可能通过免疫调节方式诱导EOS腺上皮细胞凋亡、坏死而起作用。(4)XCHT对EMS大鼠EOS生长增殖的抑制作用与丹那唑的作用机理不尽相同。
Objective: To investigate the effects of Xiao Chai Hu Tang (XCHT) on endometriotic tissue (EOS) and explore the probable mechanisms by means of the Endometriosis (EMS) rat model.
    Methods: (1) The rat EMS model established by surgical operation is divided into groups to give different concentration XCHT and Danazol to cure 4 weeks. (2) By observing and measuring EOS' volume before and past treatment, to explore the inhibition of XCHT and Danazol. (3)height of endometrial epithelium(H) , volume of glandular nucleus(V) and volume integrated optical density(Viod) of glandular nucleus in EOS were investigated by the morphometry method.(4) Ultrastructural change in EM and EOS of each group were observed by electron microscopy. (5)Caspase-3 and Fas protein expression in EM and EOS of each group were observed and analyzed by immunohistochemistry method.
    Results: (1) The rat models of surgically induced endometriosis was established successfully.(2)Compared with control group , the EOS'volume of middle or high-concentration XCHT and Danazol group was obviously reduced(P<0.01).(3)the diminution of the H and V and Viod in the middle or high-concentration XCHT and Danazol groups were more significant than the control group(P<0.01).(4) the glandular cell in the endometrial implant after therapy with middle or high-concentration XCHT showed characteristic features of apoptosis which displayed by the cell decreased in size, karyopyknosis , cytoplasmic and nuclear chromatin condensation , density increased and a lot of apoptotic bodies among cell. Whereas some stromal cell displayed degeneration and necrosis. (5)The protein expression of Caspase-3 and Fas in control group's EM were higher than it EOS; while XCHT group was on the other way round and the difference was not obvious in Danazol group.
    Conclusions: (1)The abnormal apoptosis level play a important role in Endometriosis (2)XCHT could inhibit the growth and proliferation of EOS in rat EMS model. (3) The probable mechanisms that XCHT inhibit EMS was that cell apoptosis and necrosis had induced by XCHT immuno-modulation.
