健脾开胃颗粒对胃肠功能的影响及机理探讨

摘要
目的:通过健脾开胃颗粒对脾虚大鼠胃动素(MTL)、胃泌素(GAS),胃酸、胃蛋白酶分泌、胃排空的影响,及对小鼠小肠推进运动、排便频率影响的试验,探讨健脾开胃颗粒对胃肠功能的影响及机理。研究一种疗效高、剂量少、毒性少的新型健脾开胃药物。
     方法:用人参、白术(麸炒)、茯苓、山楂、麦芽、神曲、陈皮、甘草等八味中药组成的颗粒剂,给SD大鼠灌胃,除空白组外,其余各组灌200%的大黄液(4ml/只/次/天×14天)造脾虚模型,连续14天,从第15天起空白组和模型组继续灌水,给药组分别灌39.6%、79.2%、158.4%的健脾开胃药液,阳性组对照48.6%的江中健胃消食片药液,分别观察药物对脾虚试验大鼠MTL、GAS,胃酸、胃蛋白酶分泌、胃排空的影响;除空白组外,其余各组用100%的生大黄药液0.8mL/只/次/天×8天,造成小鼠脾虚模型后,空白组和模型组均灌水,给药组灌28.6%、57.2%和114.4%的健脾开胃药液,阳性对照灌35.1%江中健胃消食片药液,灌胃容积均为0.2mL/10g,分别观察药物对脾虚小鼠小肠推进运动、排便频率的影响。
     结果:模型对照组与空白对照组比较差异非常显著(P<0.01),说明脾虚模型大鼠的血浆MTL、血清GAS水平显著降低,我们观察到脾虚模型给药组(低、中、高剂量)、阳性组经治疗后,血浆MTL、血清GAS水平都不同程度地得到恢复。高剂量的健脾开胃颗粒可使下降的脾虚大鼠血浆MTL、血清GAS基本回升到正常水平,与模型组比较。模型对照组与空白对照组比较,胃酸分泌减少,胃蛋白酶活性降低,影响了胃的正常消化功能。健脾消食类的药理作用之一与其调节胃酸、胃蛋白酶的分泌作用有关,为临床选择性用药以及分析药物作用机制提供必要的依据。给药组、阳性组通过增加胃酸分泌,提高胃蛋白酶活性而改善胃功能。同时空白组、给药组、阳性组胃排空时间也大大少于模型组,主要使健脾消食药提高了脾胃的消化、吸收、运输营养物资和水分的功能,调节脾虚小鼠小肠推进速度,并呈剂量依赖关系,使其接近空白组正常的小肠推进速度及排便频率。
     结论:健脾开胃颗粒通过调节脾虚大鼠血浆MTL、血清GAS分泌,增加脾虚大鼠的胃液、胃酸总量,提高胃蛋白酶的活性,增强脾胃的运化、腐熟、受纳功能,加强脾虚大鼠的胃排空和胃肠运动,调节小肠的推进速度和消化吸收功能,并且随着药物剂量的加大,对脾虚大鼠的胃肠运动成正相关系。由此可见,健脾开胃颗粒确实有健脾和胃,消食化积的功能,对治愈众多的因脾胃虚弱而引起的消化不良症,具有重大意义。
Purpose: Through the influence of the spleen and appetite whetted pellet to the big mouse MTL, GAS which has spleen deficient, the gastric acid, the pepsin secretion, and the stomach emptying, and through the experiment that the influence of the small intestine's propulsive movement and the defecating frequency to the small mouse, the paper discussed the influences and mechanism of the spleen and appetite whetted pellet to the gastrointestinal function. Then one kind of the spleen and appetite whetted medicine which is curative effect high, the dosage few, the toxicity is few is studied.
     Methods: Using the soup of the eight tastes Chinese native medicines, namely the ginseng, the baishu(bran fries), the fuling, the Chinese hawthorn, the malt, the shengqu, the orange peel, and the licorice to replace granule. Fills the stomach for the SD big mouse, besides the blank group, other each group fills 200% big yellow liquor (4ml/one/times/day×14 days) to make spleen deficient models, continuously 14 days; Continue to irrigate its blank group and the model group from the 15th days on; Fill the spleen and appetite whetted medicine fluid of 39. 6per , 79.2 per, 158.4 per respectively for the medicine component, and compares of the stomach digestion medicine fluid of 48.6per for the masculine group; At last, separately observes the influences of the medicine to the experimental big mouse MTL, GAS, the gastric acid, the pepsin secretion, the gastric emptying, and the propulsive movement, the defecating frequency to small intestine of the mouse to six groups.
     Results: The comparison difference of model comparison group and blank comparison group is extremely remarkable (P"0.01"), which explained the blood plasma MTL and the blood serum GAS level of the spleen deficient mouse existence have remarkably reduced. We observed after the treatment of the spleen deficient model (to be low, center, high dosage) for the medicine group and the masculine group, the blood plasma MTL and the blood serum GAS level obtains the varying degree restoration. The high dosage is good for the spleen and appetite whetted pellet to be possible to cause the dropping spleen deficient big mouse rise again to the normal level blood plasma MTL, blood serum GAS basically. After the comparison of the model comparison group and the blank comparison group, the gastric acid secretion reduces, pepsin activeness reduced, which has affected the normal digestion function of the stomach. One of pharmacological actions of the spleen digestion is related to its adjustment of gastric juice and the pepsin secretion, which provides the essential basis for the clinical selectivity with the medicine as well as the analysis of medicine function mechanism. For the medicine group and the masculine group, enhance the pepsin activeness to improve the stomach function through the increase of gastric juice secretion. Simultaneously, for the blank group, the medicine group and the masculine group, the time of stomach emptying also greatly is short to the model group. Mainly using the spleen whetted and digestion medicine to enhance the taste digestion, the absorption, the transportation nutrition commodity and the moisture content function of the spleen and stomach, to adjust propulsive speed of the spleen deficient mouse' small intestine and assumed the dosage dependence relations, and enable it to approach the propulsive speed of the blank group's normal small intestine and the bowel movement frequency.
     Conclusion: The spleen and appetite whetted the pellet, through adjustment of the spleen deficient big mouse's blood plasma MTL, the blood serum GAS secretion, increases the spleen deficient mouse's gastric juice, total quantity of the gastric acid, enhances the activeness of pepsin, boosts up the transports, well-rotted compost, and absorbing function of the gastro spleen, strengthens the spleen deficient big mouse's stomach emptying and the gastrointestinal movement, adjusts the propulsive speed and the digestion absorption function of the small intestine, and along with medicine dosage enlarging, becomes the direct ratio with the spleen deficient big mouse's gastrointestinal movement. Thus it can be seen, the spleen and appetite whetted pellet truly is good for the spleen and the stomach, has the function of digestion, and can cure multitudinously diseases because of the spleen and appetite deficient. And it also can enhance the human body immunity and the disease-resistant ability, which has the great significance for making people be in good health, the body strong, and the vitality to be sufficient.
