COX-2在子宫内膜异位症保守性手术后病变复发中的意义

摘要
目的本文旨在检测环氧合酶-2(COX-2)在子宫内膜异位症卵巢巧克力囊肿中的表达,探讨COX-2在子宫内膜异位症保守性手术后病变复发中的意义。
     方法采用PicTure~(TM)通用试剂(PV-6000)免疫组织化学二步法检测行保守性手术的子宫内膜界位症患者复发组(25例)和未复发组(26例)术后病理标本卵巢巧克力囊肿中COX-2的表达。卵巢单纯性囊肿患者为对照组(10例)。
     结果1.空白对照切片中未见阳性染色细胞,阳性对照结肠癌切片中腺癌细胞可见阳性染色。
     2.COX-2在研究组子宫内膜异位症组织中阳性表达率为7d。5%,在对照组组织中阳性表达率为30.00%,两者的表达差异有显著性(P=0.0104,<0.05)。
     3.COX-2在复发组中阳性表达率为60.00%,未复发组中阳性表达率为88.46%,两者的表达差异有显著性(P=0.0266,P<0.05)。
     结论1.COX-2在行保守性手术的子宫内膜异位症患者复发组和未复发组中均呈高表达。
     2.COX-2高表达者,临床预后好,复发风险明显降低。
     3.COX-2可能参与了子宫内膜异位症的发生发展和复发的过程。
     4.对子宫内膜异位症患者手术后病理标本进行COX-2表达的监测,可为子宫内膜异位症复发机制的研究提供理论依据,为子宫内膜异位症术后复发的治疗提供新思路。
Objective We detect the expression of eyclooxygenase- 2(COX-2)in chocolate cysts of ovary of endometriosis in order to study the significance of COX-2 in the recurrence of endometriosis after conservative surgery.
     Methods The patients with endometriosis received first conservative surgery were divided into the recurrent group of twenty-five cases and unrecurrent group of twenty-six cases.The control group is of ten simple cysts of ovary.The expression of COX-2 in chocolate cysts of ovary and simple cysts of ovary was all detected by two-step PicTure~(TM)(PV-6000 ) immunohistochemistry staining methods.
     Results 1.No positive staining cells were detected in the blank control section.The adenocarcinoma cells of positive control section of colon carcinoma were found positive staining.
     2.The positive expression of COX-2 was 74.5%in the study group and 30.0%in the control group. The expression of COX-2 in the study group was significantly higher than that in the control group(P =0.0104,P<0.05).
     3.The positive expression of COX-2 was 60.0%in recurrent group and 88.46%in the unrecurrent group.The expression of COX - 2 in the unrecurrent group was significantly higher than that in the recurrent group(P=0.0266,P<0.05).
     Conclusion 1.The expression of COX-2 showed both higher positive in the recurrent and unrecurrent groups.
     2.COX-2 overexpression characterized the patients with endometriosis who underwent conservative surgery with lower risk of relapse.
     3.COX-2 might be involved in the pathogenesis and progress and relapse of endometriosis.
     4.It may provide theoretical basis of researching on the mechanism of the recurrence of endometriosis in monitoring the expression of COX-2 in pathology specimen after surgery.The study of COX-2 may provide a new idea of clinical treatment to the recurrence of endometriosis.
